Alignment Health is looking for an Advanced Practice Clinician, NP/PA to join our Care Anywhere (CAW) program. In this role, you will provide a critical level of medical and social support to our patients in the comfort of their own homes. Your work will focus on preventing unnecessary hospitalizations, health complications, and unmanaged disease progression.
What You'll Do
- Conduct in-home assessments, medication reviews, and health screenings for members
- Provide patient education by aligning care desired with best care given
- Identify diagnoses for assessment in care management and active medical management
- Partner and communicate with the Regional Medical Officer and team members to develop appropriate care plans
- Coordinate care with multiple stakeholders, including PCPs, specialists, and ancillary providers
- Lead a broader clinical team which may include nurses, health coaches, social workers, and care coordinators
What We're Looking For
- Master’s degree from an accredited NP Program or PA program
- Ability to communicate positively, professionally and effectively with others
- Effective problem solving, organizational and time management skills
- Ability to work in a fast-paced environment
- Active Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number (NP only)
- Active Nurse Practitioner Board Certification or Physician Assistant Certification
- NPI Number, DEA, and Valid BLS certification
- Valid Driver License and current automobile insurance
- Must be able to conduct home visits in Placer County
Nice to Have
- One (1) year of prior clinical or home care experience
- Previous EMR experience
- Experience in care of older adult (geriatric) patients

Benefits & Compensation
- Compensation: $130,332.00 - $195,498.00

This role involves conducting local home visits in Placer County.
