Nova Scotia Health is looking for an Access Navigator (Preschool Autism Resource Coordinator) to join the Clinical Intake & Care Coordination team. In this role, you will act as the single point of contact for clients, families, and service providers at the point of referral into the Provincial Preschool Autism Service (PPAS). Your core focus will be to explore child and family needs and assist in matching them with available services.
What You'll Do
- Be the first point of contact for families contacting the Provincial Preschool Autism Service (PPAS).
- Explore child and family needs upon entry into PPAS and review their goals and priorities as needs change.
- Assist in matching child and family needs with the services available across PPAS streams.
- Interact with clinicians and administrative staff within PPAS foundational education, intervention, and diagnostic streams to support a coordinated continuum of care.
- Interact with community-based organizations, partners, and clinicians involved in providing autism services.
Team & Environment
You will be part of the Clinical Intake & Care Coordination stream within Provincial Preschool Autism Services. You will report to the assigned PPAS manager and be supervised by the Clinical Leader, Clinical Intake and Care Coordination.
