About the Role

Lehigh Valley Health Network is hiring an Access Center Triage RN to provide critical triage telephone services, ensuring prompt identification of patients with high-risk conditions. What You'll Do

  • Direct callers to the appropriate level of care and provide healthcare advice and education using assessment skills, approved guidelines, and knowledge of self-care practices.
  • Provide clinical support in the assessment of symptoms, determination of symptom acuity, offering information for symptom management, and patient education.
  • Facilitate coordination of care across settings for a designated population, ensuring labs, studies, and referrals are completed and results are discussed with the patient.
  • Perform a short evaluation of the patient situation to estimate illness or injury severity, including chief complaint and mental status information.
  • Serve as a liaison between patients, physicians, hospital staff, programs, and other resources.
  • Refill prescriptions and place orders for appropriate testing and labs.
  • Assist in medical record maintenance and accurately file patient charts and reports.
  • Prepare documentation and transmit medical information.

What We're Looking For

  • Associate’s Degree in Nursing.
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania.
  • 5 years of experience in nursing, including acute care, ED, medical office, or telephone triage.
  • 2 years of previous clinical acute care hospital or urgent care experience.
  • American Heart Association Basic Life Support certification for the State of Pennsylvania.
  • Excellent computer skills and empathy for individuals seeking healthcare information.
  • Knowledge of telephone-based clinical assessment techniques and medical practice telephone triage protocols.
  • Skill in making triage decisions and responding quickly and calmly in emergency situations.
  • Knowledge of appropriate home health information for patients.
  • Flexibility to perform a variety of tasks in a fast-paced environment.
  • Judgement in dealing with sensitive, confidential information.

Nice to Have

  • Bachelor’s Degree in Nursing (BSN).

Work Mode

This is a remote position requiring residency in Pennsylvania.
