As a Supervisor, LTSS Clinical Care Management at Centene Management Company LLC, you will lead the care coordination team serving long-term care members with complex medical and behavioral health needs. Your work is central to promoting the quality and effectiveness of our care management delivery. This is a remote position requiring residency in Delaware, with up to 40% field-based work.
What You'll Do
- Supervise day-to-day escalations and care management issues related to members or providers.
- Monitor and review long-term care management documentation to ensure compliance with federal and state regulations and contracts.
- Assign caseloads and work assignments based on state requirements, staff experience, and member needs.
- Work with senior management on escalated, complex cases and provide guidance to junior team members.
- Educate and provide resources for the care management team on key initiatives and member outreach.
- Evaluate team performance and provide feedback on goals and career milestones.
- Provide coaching and guidance to the care management team and providers to ensure high-quality member care.
- Monitor reports regularly to ensure quality and productivity metrics are met.
- May perform telephonic, digital, home, or other site outreach to assess member needs and collaborate with resources.
- Assist with onboarding, hiring, and training of long-term care management team members.
- Lead and champion change within your scope of responsibility.
What We're Looking For
- Must reside in the state of Delaware.
- For positions overseeing RN team members: Graduate from an Accredited School of Nursing or a Bachelor's degree and 4+ years of related experience.
- For positions overseeing RN team members: Current state's Registered Nurse (RN) license required upon hire.
- For positions overseeing BH team members: Master's degree or Graduate from an Accredited School of Nursing and 4+ years of related experience.
- For positions overseeing BH team members: Licensed Clinical Behavioral Health Professional or RN based on state contract requirements (e.g., LCSW, LMSW, LMFT, LMHC, RN with BH experience) required upon hire.
Nice to Have
- Highly advanced clinical knowledge and ability to assess member needs in context of relevant diagnoses, treatment plans, and goals.
- Ability to identify potential gaps in care or risks for readmission or complications.
- Strong knowledge of healthcare managed care principles.
- Experience working with providers and healthcare teams to develop appropriate long-term care plans.
- Strong knowledge of medication indications and side effects and/or knowledge of behavioral health care management principles.
