What You'll Do
Guide a team of Dialysis Transition Managers dedicated to streamlining the movement of renal patients from hospital settings to outpatient care. You'll shape outreach strategies, refine processes, and ensure a smoother experience for both patients and healthcare providers. Your leadership will focus on continuous improvement, using data and stakeholder feedback to drive meaningful change.
Collaborate across departments to identify high-impact opportunities and lead cross-functional efforts from concept to execution. You'll translate complex challenges into clear, actionable plans and present insights to senior leaders with confidence. By building strong connections with hospital staff and internal teams, you’ll strengthen DaVita’s role as a trusted partner in patient care transitions.
Develop team members through coaching and skill-building, fostering an inclusive environment where individuals can grow and thrive. You’ll manage project timelines, allocate resources effectively, and ensure accountability across initiatives. Regular outreach to discharge planners and coordination with clinic teams will help optimize patient placement and improve overall service delivery.
Requirements
- Proven experience in leading teams and delivering measurable improvements in operations or financial outcomes
- 5–10 years of progressive work experience, with a track record of managing change from planning through implementation
- Ability to travel up to 30%, typically 5–10%, to support hospital and clinic engagements
- Strong communication and interpersonal skills to influence diverse stakeholders and present to executive audiences
- Skilled at simplifying complex issues into clear, compelling narratives
- Comfortable balancing strategic thinking with hands-on execution
Preference will be given to candidates with a background in healthcare or a related field.
