Nodaway County Services is hiring a Service Coordinator to deliver targeted case management that improves quality of life for individuals with developmental and intellectual disabilities. In this role, you will develop person-centered plans, link individuals to vital resources, and advocate on their behalf to achieve meaningful outcomes while ensuring compliance with all applicable regulations.
What You'll Do
- Identify, utilize, and maintain positive relationships with community resource providers relevant to individual needs and goals.
- Engage all parties in developing person-centered plans and selecting appropriate supports, services, and providers, documenting all plans and updates.
- Establish clear communication to build beneficial partnerships with individuals, families, service providers, public officials, and associates.
- Monitor service effectiveness and contractual compliance, completing all necessary documentation, reports, and correspondence in a timely manner.
- Promote continuous opportunities for individuals in key outcome areas: Daily Life, Community Living, Social Spirituality, Healthy Living, Safety and Security, Citizenship and Advocacy, and Supports for Families.
- Advocate for individuals, protect their rights, and serve as a mandated reporter to ensure health and safety while maintaining appropriate professional boundaries.
- Comply with HIPAA (Health Insurance Portability and Accountability Act of 1996) confidentiality regulations.
- Apply a working knowledge of the contractual and legal requirements for TCM services.
- Engage in ongoing professional development and training opportunities.
What We're Looking For
- Experience in case management, support coordination, or a related human services field.
- Demonstrated ability to develop, implement, and monitor person-centered plans.
- Strong relationship-building and communication skills for interacting with diverse individuals, families, and community partners.
- Understanding of advocacy principles, rights protection, and the role of a mandated reporter.
- Knowledge of local, state, and federal regulations governing developmental disability services.
- Commitment to maintaining confidentiality and understanding of HIPAA requirements.
- Proficiency in thorough and timely documentation and report writing.
- Ability to empathize with a wide range of backgrounds, experiences, beliefs, and needs.
Team & Environment
You will report directly to the Service Coordination Program Manager and collaborate closely with a dedicated team focused on supporting individuals and their families.
