About the Role

Highmark Inc. is seeking a Senior Pharmacist - Care Management to join a team-oriented environment focused on improving member engagement and outcomes. In this role, you will act as an adjunct to health coaching, developing educational initiatives and participating in cross-functional clinical development.

What You'll Do

  • Work one-on-one with members via face-to-face and telephonic interactions.
  • Develop individual care plans, coordinate care, and assist in discharge planning.
  • Represent the organization to members, providers, and practice staff.
  • Assist in developing and implementing utilization, disease, and chronic condition management programs.
  • Address gaps in care, medication adherence, and perform medication reconciliation.
  • Collaborate to promote appropriate, cost-effective medication use per formulary and utilization requirements.
  • Support initiatives to reduce emergency department utilization.
  • Develop and implement educational initiatives for physicians, pharmacies, and members.
  • Participate in cross-functional committees to develop clinical and operational initiatives.
  • Identify and assist in managing department process improvements for efficiency.

What We're Looking For

  • Bachelor of Science in Pharmacy or a Doctor of Pharmacy degree.
  • 5 - 7 years of post-graduate pharmacy experience.
  • Registered Pharmacist license.

Nice to Have

  • 1 - 3 years of progressive clinical experience in ambulatory care.
  • Experience with Medicaid populations.
  • Post Graduate Year 1 (PGY1) residency with concentration in ambulatory care.
  • Board Certified Pharmacotherapy Specialists (BCPS) certification.
  • Board Certified Ambulatory Care Pharmacist (BCACP) certification.

Benefits & Compensation

  • Salary: $78,900.00 - $147,500.00

This role requires an onsite work mode.
