Highmark Inc. is seeking a Senior Pharmacist - Care Management to join a team-oriented environment focused on improving member engagement and outcomes. In this role, you will act as an adjunct to health coaching, developing educational initiatives and participating in cross-functional clinical development.
What You'll Do
- Work one-on-one with members via face-to-face and telephonic interactions.
- Develop individual care plans, coordinate care, and assist in discharge planning.
- Represent the organization to members, providers, and practice staff.
- Assist in developing and implementing utilization, disease, and chronic condition management programs.
- Address gaps in care, medication adherence, and perform medication reconciliation.
- Collaborate to promote appropriate, cost-effective medication use per formulary and utilization requirements.
- Support initiatives to reduce emergency department utilization.
- Develop and implement educational initiatives for physicians, pharmacies, and members.
- Participate in cross-functional committees to develop clinical and operational initiatives.
- Identify and assist in managing department process improvements for efficiency.
What We're Looking For
- Bachelor of Science in Pharmacy or a Doctor of Pharmacy degree.
- 5 - 7 years of post-graduate pharmacy experience.
- Registered Pharmacist license.
Nice to Have
- 1 - 3 years of progressive clinical experience in ambulatory care.
- Experience with Medicaid populations.
- Post Graduate Year 1 (PGY1) residency with concentration in ambulatory care.
- Board Certified Pharmacotherapy Specialists (BCPS) certification.
- Board Certified Ambulatory Care Pharmacist (BCACP) certification.

Benefits & Compensation
- Salary: $78,900.00 - $147,500.00
