SSM Health is hiring a RN-Virtual Health, Clinic to provide remote patient care under the direction of a qualified health care provider. You will function within the scope of your license to support different clinics and specialty clinics, while also participating in program development and process improvement.
What You'll Do
- Complete nursing workflows per department protocols including virtual/telephonic patient program admission, assessments, education, care planning, coordination, and remote patient monitoring (RPM) escalation follow-up.
- Deliver safe and quality care in line with provider orders, remote patient monitoring department protocols, and established nursing care standards.
- Provide documentation that supports the treatment plan, coordinates patient care, meets regulatory requirements, and ensures reimbursement.
- Communicate with the management team, patient care team (including clinical staff and providers), and patient/patient caregiver(s) per department protocols.
- Use electronic technology for data collection, documentation, information gathering, and communication.
- Apply evidence-based practice and scientific knowledge to ensure nursing care is delivered based on patient’s age-specific and clinical needs as described in the department's scope of service.
- Work in a constant state of alertness and in a safe manner.
What We're Looking For
- Graduate of an accredited school of nursing or education equivalency for licensing.
- One year of registered nurse experience.
- For an Illinois work location: Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
- For a Missouri work location: Registered Nurse (RN) - Missouri Division of Professional Registration OR a Registered Nurse (RN) Issued by Compact State.
- For an Oklahoma work location: Registered Nurse (RN) Issued by Compact State OR Registered Nurse (RN) - Oklahoma Board of Nursing (OBN).
- For a Wisconsin work location: Registered Nurse (RN) Issued by Compact State OR Registered Nurse (RN) - Wisconsin Department of Safety and Professional Services.
