Baptist Health is seeking a dedicated RN Hospice On Call to join our team. You will be responsible for the systematic collection of patient-specific data to determine individualized care plans, providing compassionate end-of-life support and education.
What You'll Do
- Systematically collect patient-specific data necessary to determine patient plan of care.
- Document patient data and plan of care accurately and promptly.
- Teach and counsel patients and families regarding their care.
Work Mode
This is an onsite position based at the Baptist Health Resource Center.
