The Case Manager plays a critical role in our healthcare team, collaborating to enhance patient care services across the entire care continuum. By leveraging broad clinical and systems knowledge, case managers evaluate, predict, and facilitate patient care trajectories. Through a comprehensive approach of assessment, planning, implementation, coordination, monitoring, advocacy, and evaluation, we meet individual healthcare needs efficiently and effectively.

Qualifications
Requirements for Case Management RN:
- RN Case Manager experience required
- Massachusetts RN license
- Minimum 2 years Med/Surg experience
- Case Manager certificate preferred
- ADN required, BSN strongly preferred
- Acute hospital experience preferred
