Mass General Brigham seeks a dedicated RN Case Manager for its Palliative Care team at Massachusetts General Hospital. You will collaborate with the healthcare team to enhance patient care delivery along the continuum, focusing on patient needs, quality care, cost reduction, and reimbursement. This position includes an in-depth orientation and support from leaders and subject matter experts.

What You'll Do

• Collaborate as a member of the health care team to enhance the delivery of patient care services along the continuum of care.
• Meet patient’s needs efficiently and expeditiously by continuously improving the patient’s experience.
• Help ensure institutional standards of high-quality patient care, reduce cost, and ensure reimbursement.
• Evaluate, predict, and facilitate the trajectory of patient care through knowledge of clinical care and systems management.
• Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet individual health care needs.

What We're Looking For

• Current Massachusetts Registered Nurse (RN) State License.
