Mass General Brigham is hiring a per diem Case Management Assistant to provide essential support for inpatient transition planning. In this onsite, acute care role, you will collaborate closely with the care team to coordinate resources for complex patients and ensure the efficient utilization of healthcare services.
What You'll Do
- Support the Case Manager in coordinating, negotiating, procuring, and managing resources to support the care of complex patients.
- Facilitate achievement of quality care and patient cost outcomes.
- Work collaboratively with interdisciplinary staff internal and external to the organization.
- Ensure efficient utilization of health care resources including acute, intermediate, and chronic care services, specialty programs, and services.
- Participate in quality improvement and evaluation processes related to the management of patient care.
- Work independently and prioritize tasks.
- Meet and interview patients in an acute care setting.
- Interact with patients and members of the care team.
- Support compliance with applicable criteria, guidelines, and regulatory requirements for patient management.
- Work with Case Manager to manage available resources to facilitate optimal outcomes for targeted patient groups.
- Assist Case Manager in implementing a comprehensive framework for case management to promote wellness and maximize reimbursement.
What We're Looking For
- One or more years of experience in a service or healthcare industry environment.
- High school diploma or equivalent.
- Ability to work independently and prioritize tasks.
- Good communication and excellent interpersonal skills.
- Computer literate, with the ability to quickly gain competency in various hospital software applications.
- Ability to operate telephone equipment, fax machine, copy machine, and calculator.
- Ability to simultaneously track and coordinate multiple details.
- Ability to discern important details and facts from fast-paced conversation and available documentation.
- Good critical thinking skills and a creative, organized approach to all work tasks.
- A mature professional with a high level of personal energy and professional motivation.
- Knowledge of payer requirements, vendor services, and the systems and processes related to obtaining those services.
- A commitment to cost-effective patient care and patient education.
- Ability to work independently within a team-oriented, structured environment.

Benefits & Compensation
- Compensation: $17.36 - $24.79 per hour.
Work Mode
This is an onsite role located at 81 Highland Avenue.
