Henry Ford Providence Hospitals is looking for an RN Case Manager to join our team on a contingent (as needed) basis at Henry Ford Providence Southfield Hospital. In this day shift role, you will be responsible for creating care plans across the continuum, monitoring patient medical necessity, and advocating for essential resources.
What You'll Do
- Create plans for care across the continuum, integrating patient and family preferences and values.
- Monitor patient medical necessity and level of care through assessments, ongoing evaluations, and patient records.
- Advocate for resources and work to remove barriers to care.
- Maintain ongoing dialog with your supervisor and care transition team members to ensure effective health plan implementation and reevaluation.
- Act as a resource for adequate medical record documentation, level of care recommendations, and post-discharge care services related to diagnoses and treatment options.
What We're Looking For
- A diploma from an accredited school or college of nursing OR the required professional licensure at the time of hire.
- A Registered Nurse credential from the Michigan Board of Nursing obtained prior to hire or job transfer.
Nice to Have
- A BSN is preferred.
- Acute care case management experience is preferred.
- Experience with ECIN/Allscripts/Careport, Google, Outlook, and Microsoft products is preferred.
- A BLS Provider credential, accepted from the American Heart Association, is preferred.
- A Case Manager credential from the Commission for Case Manager Certification (CCMC) is preferred.
- A Case Manager credential from the American Case Management Association (ACMA) is preferred.
