CVS Health is hiring a Retail Health Coordinator to join our team at a HealthHUB location in Loganville, GA. In this role, you will serve as a crucial link between our members and their healthcare journey, working from the Wellness room to proactively engage patients, coordinate care, and help close important health gaps. You'll leverage technology to connect telephonically and ensure a seamless patient experience.
What You'll Do
- Communicate telephonically with identified members about open healthcare gaps.
- Coordinate patient flow throughout all scheduled appointments.
- Interact with customers via phone, video, email, and text to deliver healthcare reminders.
- Schedule, track, and reconcile patient referrals and diagnostic tests.
- Use the Scheduler app per written workflows and document interactions.
- Demonstrate proficiency with Microsoft Office, remote communication software (Skype, Teams, WebEx), and documenting in an electronic health record like EPIC.
- Adhere to care management, privacy, confidentiality, and quality processes.
- Attend weekly team huddles to stay connected with field operations.
- Provide superior customer service to all internal and external contacts.
- Communicate patient care plans with the clinical team, patients, and families.
- Assist with completing and tracking patient forms and letters.
- Notify the clinical care team of any changes or barriers.
- Cross-train to other areas of retail health management.
- Participate in orientation and training of staff as assigned.
- Demonstrate respect and regard for the dignity of all patients, families, visitors, and colleagues.
What We're Looking For
- High School Diploma or equivalent.
- Minimum of 3-5 years of experience in healthcare, customer service, patient relations, or hospitality concierge.
- Naturally passionate about engaging members telephonically to improve health outcomes.
- Comfortable in a dynamic role open to continuous growth; able to receive and act on feedback.
- Strong computer, data entry, and typing skills with the ability to work across multiple technology platforms.
- Proficiency in Microsoft Office applications, including Outlook, Word, and Excel.
- Strong organizational skills and attention to detail; ability to multitask and prioritize.
- Ability to interact appropriately with others and maintain a pleasant demeanor.
- Self-starter able to work successfully with minimal supervision.
- Great problem solver with collaborative engagement skills.
- Understanding of health insurance, including local providers, general plan types, and terminology.
- Able to read instructions, reports, and information on computer screens.
- Good judgment, interpersonal skills, and a strong level of confidentiality.
- Excellent time management, customer service, and communication (verbal, written, listening) skills.
Nice to Have
- Associate's Degree.
- Certification/Licensure such as Medical Assistant or Certified Pharmacy Technician (CPhT).
- Work experience in a doctor's office or in coordination of care.
- Experience in a call center or telephonic care management setting.
- Prior experience handling sensitive information confidentially, including PHI protected under HIPAA.
- Experience with electronic medical records, e.g., Epic.
- Bilingual (Spanish).
- One year of community-based experience within an interdisciplinary care team.
