CareHarmony is hiring a Remote Triage Nurse for a PRN weekend shift. You will provide exceptional after-hours patient care, including telephonic triage for complex patients and assistance with psychosocial needs. This role reports to the Lead Triage RN and is central to our mission of changing how patients manage their health.
What You'll Do
- Conduct thorough telephonic patient assessments using proprietary tools.
- Identify and address patient concerns, including acute clinical issues in close collaboration with providers.
- Provide telephonic education to patients.
- Ensure excellent quality of care during all on-call and triage interactions.
- Participate in Innovation team projects.
- Foster positive relationships within the team to support a positive and supportive culture.
What We're Looking For
- Current, active Compact/Multistate Nursing Licensure in good standing.
- Graduate of an accredited School of Nursing.
- A minimum of five (5) years of professional nursing experience.
- Extensive experience in telephonic triage is required.
- Robust clinical knowledge and ability to quickly determine the appropriate level of care.
- Excellent verbal, written, and customer service skills with a strong professional presence.
- Excellent attention to detail and organizational skills.
- Ability to navigate multiple technological platforms.
- Ability to function cohesively within a team.
- Innovative mindset, driven to improve patient health management.
- Must have active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.
Nice to Have
- Additional state nursing licensures.

Benefits & Compensation
- Compensation: $25/hr for LPN / $27/hr for RN.
