About the Role
Deliver high-quality, patient-centered care through virtual consultations, using clinical expertise to assess, diagnose, and manage acute and chronic conditions. Support ongoing patient education and coordinate care as needed.
Responsibilities
- Conduct virtual patient evaluations and follow-up visits
- Diagnose and treat acute and chronic medical conditions
- Develop individualized care plans based on patient needs
- Order and interpret diagnostic tests
- Prescribe medications in accordance with regulations
- Maintain accurate and timely electronic health records
- Provide patient education on disease management and prevention
- Collaborate with care teams to ensure continuity of care
- Monitor patient progress and adjust treatment plans
- Adhere to clinical protocols and safety standards
- Ensure compliance with state and federal healthcare regulations
- Respond to patient messages within established timeframes
- Support preventive health initiatives
- Use evidence-based guidelines in clinical decision-making
- Participate in quality improvement activities
