Deliver high-quality virtual care to patients by conducting assessments, diagnosing conditions, and managing treatment plans using clinical guidelines and individualized approaches.
Responsibilities
- Conduct comprehensive patient evaluations through virtual platforms
- Diagnose and treat acute and chronic medical conditions
- Develop and adjust individualized care plans based on patient needs
- Order and interpret diagnostic tests as clinically appropriate
- Prescribe medications following state and federal regulations
- Maintain accurate and timely electronic health records
- Collaborate with care coordination teams to support patient outcomes
- Provide patient education on disease management and wellness
- Ensure compliance with clinical protocols and safety standards
- Engage in ongoing quality improvement initiatives
- Respond to patient messages within defined response timeframes
- Participate in team huddles and clinical meetings
- Stay current with medical advancements and treatment guidelines
- Support patients through transitions of care
- Monitor patient progress and adjust treatment as needed
Nice to Have
- Experience in women's health or menopause care
- Background in endocrinology or hormonal health
- Prior telehealth or virtual care experience
