CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) with an active Oregon license to join our team delivering value-based care management. In this fully remote day shift role, you will work comprehensively with providers to manage a patient census, coordinate community and clinical resources, and provide essential patient education.
What You'll Do
- Manage your patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
- Identify and coordinate community resources that would benefit your patients' care plans.
- Provide patient education and improve health literacy around chronic condition management.
- Perform medication management, including reconciliation, adherence monitoring, and coordinating refills.
- Assist in ensuring timely delivery of services such as Home Health, DME, and Home Infusion.
- Resolve patient questions and create an open dialogue to understand their needs.
- Assist with managing referrals and appointment scheduling.
What We're Looking For
- Active Oregon LPN license.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Experience using Epic EHR systems.
- Bilingual language skills.
- Additional state LPN licensures.
