About the Role

CareHarmony is hiring a Remote Licensed Practical Nurse (LPN) with New York licensure to serve as a Care Coordinator. In this role, you will work comprehensively with providers to deliver value-based care management initiatives, focusing on coordinating needs and identifying resources for patients in chronic care management programs.

What You'll Do

  • Work comprehensively with providers to deliver value-based care management initiatives for patients.
  • Identify resources and coordinate needs for chronic care management patients.

What We're Looking For

  • Valid Licensed Practical Nurse (LPN) licensure in the state of New York.
  • At least 3 years of direct patient-facing work experience.
