CareHarmony is looking for a remote Licensed Practical Nurse (LPN) to join our care coordination team. In this role, you will work comprehensively with providers to deliver value-based care management initiatives. You will manage a patient census with a resolution-driven approach to close gaps in care, coordinate resources, and provide patient education.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources that would benefit your patients' care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services, including Home Health, DME, and Home Infusion.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Assist and manage referrals and appointment scheduling.
What We're Looking For
- Active Massachusetts LPN License
- At least 3 years of direct patient-facing work experience
- Technical aptitude with the Microsoft Office Suite
- Excellent written and verbal communication skills
- Must have active high-speed Wi-Fi
- Must have a home office or HIPAA-compliant workspace
Nice to Have
- Experience with Epic EHR
- Bilingual skills
- Additional state LPN licensures
