Chronic Care Staffing is seeking a compassionate Remote Care Coordinator (RN) to join our growing team. This role is ideal for a nurse who thrives in a structured, remote environment and is passionate about improving outcomes for patients managing chronic conditions.
What You'll Do
- Conduct monthly Chronic Care Management (CCM) calls and complete verbal enrollments
- Perform Health Risk Assessments (HRAs), Transitional Care Management, and Remote Patient Monitoring services
- Educate patients and families on chronic conditions, medications, and treatment goals
- Collaborate with providers and internal teams to ensure effective care coordination
- Encourage adherence to care plans and promote patient self-management
- Accurately document all communications and interventions in EHR systems
- Maintain HIPAA, CMS, and company compliance standards
- Meet productivity and call volume expectations while delivering empathetic, patient-centered care
What We're Looking For
- Must be a Registered Nurse (RN)
- Must live in Mountain Standard Time (MST) or Pacific Standard Time (PST) zone
