CareHarmony is looking for a Remote Care Coordinator (LPN/LVN) to deliver value-based care management. In this role, you'll work directly with providers to manage a patient census, coordinate critical services, and improve health outcomes.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
- Identify and coordinate community resources to benefit patient care plans.
- Provide patient education and health literacy support for chronic condition management.
- Perform medication management, including reconciliation, adherence checks, and coordinating refills.
- Ensure timely delivery of services like Home Health, DME, and Home Infusion.
- Resolve patient questions and create open dialogue to understand their needs.
- Assist and manage referrals and appointment scheduling.
What We're Looking For
- Active Compact or Multi-State LPN/LVN license.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.
Nice to Have
- Experience with Epic.
- Bilingual skills.
- Additional single state licensures.

- Compensation: $21/hr with the ability to earn up to $28/hr based on production.
