Shields Health Solutions is looking for a Remote Ambulatory Care Pharmacist to provide integrated, hands-on support to patients and physicians, improving the quality of patient care. You'll develop personalized relationships within assigned clinics to enhance medication adherence and promote evidence-based therapy management.
What You'll Do
- Provide medication management including reconciliation, dose optimization, and adherence counseling within approved protocols.
- Collaborate with interdisciplinary care teams to develop individualized, evidence-based care plans.
- Monitor and evaluate medication regimens for therapeutic goals, adverse effects, and drug interactions.
- Identify opportunities for drug therapy optimization and medication cost savings.
- Participate in quality improvement initiatives, clinical outcome studies, and process improvement projects.
- Optimize resources by providing clinical justification for complex prior authorizations and appeals.
- Collaborate with healthcare providers to ensure seamless medication management and care coordination.
- Support occasional peer-to-peer requests and perform clinical assessments and education for patients on targeted medications.
- Ensure all clinical criteria comply with FDA guidance and evidence-based literature.
- Assist with workflow optimization to enhance efficiency, productivity, and quality of care.
- Advocate for patient safety and medication literacy through education and counseling.
- Protect confidential information and update job knowledge through ongoing education and professional engagement.
- Attend team meetings and professional development activities to stay current with clinical practices.
What We're Looking For
- Doctor of Pharmacy (PharmD) degree.
- Active pharmacist license in California.
- Advanced practice pharmacist designation in California, meeting two of three specified criteria.
- 3+ years of experience as a clinical pharmacist.
- Ability to analyze information and exercise sound judgment.
- Strong verbal communication skills.
- Deep knowledge of pharmacology developments and FDA health regulations.
- Ability to manage processes and exhibit strong technical and clinical skills.
- A constant focus on legal compliance, quality, and detail.
- Strong clinical knowledge and validated expertise in disease state management and treatment guidelines.
- Organized, high-integrity, dependable, empathetic, and an excellent listener/communicator.
- Energetic, highly motivated team player with strong personal skills.
- Discretion and confidentiality essential for handling sensitive data.
Nice to Have
- Completion of a PGY1/PGY2 residency.
Team & Environment
You will collaborate closely with a hybrid ambulatory care pharmacy team onsite and report to the Senior Director of Clinical Services and Manager of Clinical Services.
Benefits & Compensation
- Compensation for California duties: $127,000 - $155,000 plus bonus.
- Compensation for duties in other states: $127,000 - $133,000 plus bonus.
Work Mode
This is a fully remote position. The role is based in Northern CA (Remote).
