Requirements
- Associate Degree in Nursing (A.D.N.) from an accredited nursing program is required; Bachelor of Science in Nursing (B.S.N.) is preferred. A three-year diploma from an accredited diploma program may be accepted in place of a degree.
- Active Registered Nurse (RN) license valid in the state of practice is mandatory.
- Must maintain continuing education credits as mandated by the state licensing board.
- A minimum of five years of clinical nursing experience is required.
- Experience in case management is required. Alternatively, a minimum of three years assessing patient needs, creating and managing individualized care plans, coordinating interdisciplinary care, and guiding patients through complex healthcare transitions will be accepted.
- Proven understanding of clinical, financial, and patient satisfaction metrics and experience managing performance toward these outcomes.
- Demonstrated ability to lead and support change initiatives effectively.
- Familiarity with Continuous Quality Improvement (CQI) methodologies and practices.
- Prior experience in healthcare performance coaching is required.
- Current CPR certification is mandatory.
- Ability to adapt personal workflows to new electronic systems and increased productivity demands related to Capella software implementation.
- Valid driver’s license in the state of employment with a clean driving record and compliance with insurance requirements is required.
- Basic computer literacy with proficiency in Microsoft Word and Outlook is required.
- Functional competence in company-specific clinical software, including Capella, must be achieved within 90 days of hire.
- A home office setup with reliable internet connection of at least 1 Mbps upload and 1 Mbps download speed is required.
Nice to Have
- Certification as a Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
- At least three years of experience in renal or nephrology nursing is preferred.

DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division focused on delivering coordinated care to patients with end-stage renal disease (ESRD), one of the most medically complex populations in the U.S. The team includes over 600 nurses, care coordinators, nurse practitioners, and business professionals who manage care for more than 20,000 patients with late-stage chronic kidney disease (CKD) and ESRD monthly across the country.

- Flexibility is necessary to meet patient needs and align with nephrology partner schedules.
- This role is primarily remote but includes 1–2 days per week in the field within an assigned territory.
- Territory assignments may be adjusted based on operational requirements.
- Service area includes Elk Grove, California, and nearby regions.
