About the Role
The nurse in this position will lead patient care management by assessing health status, developing individualized care plans, and ensuring adherence to treatment protocols. Emphasis is placed on patient education, preventive strategies, and collaboration with healthcare providers to optimize clinical results.
Responsibilities
- Conduct comprehensive patient assessments to determine care needs
- Develop and update individualized care management plans
- Educate patients and families on disease processes and treatment regimens
- Monitor patient progress and adjust care strategies as needed
- Coordinate transitions of care across healthcare settings
- Document clinical interactions and patient outcomes accurately
- Serve as a clinical resource for patients and care teams
- Promote adherence to prescribed therapies and medications
- Facilitate communication between patients and providers
- Support patients in setting and achieving health goals
- Identify and address barriers to care access and compliance
- Participate in quality improvement initiatives
- Ensure compliance with clinical policies and regulatory standards
- Conduct regular follow-ups with patients via phone and in person
- Assist in managing chronic conditions such as kidney disease
- Collaborate with social workers and other support staff
- Use electronic health records to maintain patient information
- Respond to patient inquiries and urgent clinical concerns
- Promote preventive care and wellness strategies
- Support patients through dialysis treatment phases when applicable
- Maintain patient confidentiality and ethical standards
- Attend team meetings and case reviews
- Stay current with clinical guidelines and best practices
- Contribute to patient satisfaction and retention efforts
- Support clinical onboarding and orientation when needed
