Primary Care Solutions is looking for a Registered Nurse to join a Patient Aligned Care Team (PACT) in a Community-Based Outpatient Clinic. You will provide direct healthcare to Veterans, focusing on developing and implementing patient-centered care plans in an onsite setting.
What You'll Do
- Assist practitioners with patient care and practice specialized nursing skills in patient assessment.
- Triage urgent and emergent same-day patients, identify acute needs and high acuity patients, and mobilize care teams for optimal outcomes.
- Provide support by assisting patients in navigating and coordinating their care.
- Participate in a daily team huddle.
- Answer and return patient telephone calls, messages, and alerts.
- Provide patient and family education focused on self-management, prevention, and wellness.
- Monitor discharge list and contact patients for follow up.
- Establish and maintain a warm, welcoming, and professional atmosphere for colleagues and veterans.
- Promote achievement of clinical goals and metrics.
What We're Looking For
- 1 year of Primary Care and/or Chronic Disease Management experience.
- Current, active, and unrestricted state Nursing License.
- Current BLS for providers from the American Heart Association, renewed annually.
- Strong computer skills, including EMR experience.
