Avera McKennan is hiring a Registered Nurse Case Manager (RN) for our MCK Pharm Specialty team. This pivotal role involves facilitating the pharmacy plan of care for specialty medications, working closely with clinical pharmacists, physicians, and payors to ensure proper medication use and patient adherence. You will be a key player in managing clinical outcomes and serving as a liaison to assure continuity of care, all within the framework of Avera's mission to Move Health Forward through compassion, hospitality, and stewardship.
What You'll Do
- Facilitate the pharmacy plan of care for specialty medications.
- Work closely with clinical pharmacists, physicians, and payors.
- Manage patient adherence, clinical outcomes, and payor requirements.
- Perform scheduled assessments with patients according to therapy management protocols.
- Collaborate with the interdisciplinary care team to encourage the best possible patient outcome.
- Serve as a liaison with physicians and other referral sources to assure continuity of care.
- Act as liaison with hospitals and other health care services to provide treatment advice and lobby on behalf of patients to various payor sources.
- Identify community resources for assistance with non-covered benefits.
- Provide patient care and assist providers as needed.
- Complete documentation, obtain insurance prior authorizations, and coordinate scheduling of procedures, testing, and treatments.
- Perform and assist with medical treatment teaching and follow-up care with patients and their families.
- Assist with updating all policies and procedures pertinent to the area of responsibility.
- Assist with the review and development of education materials offered to patients.
- May attend discharge planning meetings to facilitate and coordinate post-procedure needs.
What We're Looking For
- Registered Nurse (RN) with an active Board of Nursing license in your state of practice upon hire.
- Must obtain a Minnesota Nursing license within 90 days from hire.
- Must live within a 100-mile radius of Sioux Falls.
Nice to Have
- Bachelor's degree in Nursing.
- Specialty Certification in your Area of Nursing Practice upon hire.
- 1-3 years of relevant experience.

You will be an integral part of a multidisciplinary team, working collaboratively with pharmacists, physicians, and other healthcare professionals to deliver exceptional patient care.
Benefits & Compensation
- Hourly pay range: $31.00 - $46.25 per hour.
Work Mode
This is a remote, PRN position for candidates based in the Sioux Falls area.
