CircleLink Health is hiring a Registered Nurse Care Coach to provide remote Chronic Care Management for Medicare patients with multiple chronic conditions. In this role, you will make a direct impact by coaching patients to improve their health outcomes and reduce unnecessary hospitalizations.
What You'll Do
- Utilize specialized care management software to conduct monthly calls with Medicare patients.
- Build rapport with patients to coach them toward improved health using SMART goals and self-management education.
- Implement and update the Plan of Care, managing medications, appointments, biometrics, and interventions.
- Connect patients with community resources for needs like transportation, personal care, and social services.
- Conduct Transitional Care Management activities for high-risk post-discharge patients to reduce readmissions.
- Close preventive care gaps by encouraging and assisting with well visits, vaccines, cancer screens, and follow-ups.
What We're Looking For
- Fluent in English with strong telephonic communication skills.
- Self-directed and able to work independently while meeting performance metrics.
- Passion for nursing and improving patient outcomes.
- Comfort with technology and eagerness to learn new software.
- Excellent organizational and time management skills.
- Strong critical thinking and problem-solving abilities.
- Current, unrestricted Compact RN License.
- Proficiency with electronic health records and web-based applications.
- 3+ years of experience as a Registered Nurse.
Nice to Have
- Spanish fluency.
- Case Management or Chronic Disease Management experience.
- Certified Diabetes Educator credential.
- Experience with Motivational Interviewing or other behavior change techniques.

Benefits & Compensation
- Compensation: Paid at the rate of $15.00 per initial Clinical Encounter per patient per month.
- $300 performance bonus after 3 months of successful employment upon meeting expectations.
