Rockefeller Philanthropy Advisors is hiring a Referral Coordinator to join a purpose-driven environment. This fully remote role in Colorado places you at the heart of care coordination, supporting individuals on their mental health journey. You will serve as the key point of contact between clients, healthcare partners, and internal teams, ensuring timely and accurate referrals.
What You'll Do
- Coordinate referrals by interacting with healthcare systems, clients, and external partners.
- Facilitate documentation and track client progress throughout their mental health care episodes.
- Manage and route inbound and outbound communications via phone, email, and electronic systems.
- Process referral tasks accurately and on time using EMRs and related technologies.
- Conduct outreach to gather necessary information for connecting clients to appropriate care.
- Provide administrative support for post-session documentation and lead management.
- Identify and communicate problems proactively, asking questions and collaborating on solutions.
- Participate in special projects and adapt to evolving workflows and priorities.
What We're Looking For
- Bachelor's degree or equivalent experience in care coordination, healthcare, or related fields.
- Strong written and verbal communication skills and a client-centric mindset.
- Ability to adapt quickly to new technologies and shifting workflows.
- Excellent organizational skills and attention to detail in a fast-paced setting.
- Demonstrated motivation, goal orientation, and openness to feedback and development.

- Previous experience in referral or care coordination is a plus.

- Compensation: $21.64 per hour base pay.
