Equip Health is hiring a Referral Coordinator to triage incoming patient referrals from provider partners and ensure a seamless experience for patients and referrers. You will collaborate closely with our admissions and outreach teams, act as a liaison between new providers and outreach managers, and manage data across multiple systems.
What You'll Do
- Collaborate with the admissions and outreach teams to triage referred patients and support referral partners during their patient’s admissions process.
- Serve as a liaison between new providers and outreach managers.
- Manage data and reports in multiple systems including a CRM, EMR, and data analytics platform.
- Manage confidential health information thoughtfully.
- Ensure data is inputted accurately and consistently and assist with data audits as needed.
- Speak confidently about Equip’s services, patient population, and referrals process.
- Perform other duties as assigned.
What We're Looking For
- Bachelor’s degree or higher.
- 1+ years experience in a customer service or case management role.
- Excellent interpersonal and communication skills.
- Highly organized with strong attention to detail in a dynamic environment.
- Comfortable working remotely with high autonomy and self-motivation.
- Ability to think critically to quickly and accurately work through challenges and rapid change.
- A desire to work at a fast-growing health tech startup.
- Must be able to work EST time zone.
Nice to Have
- Experience preferably in healthcare.
