Responsibilities
- Partner with admissions and outreach teams to assess incoming patient referrals and assist referral sources throughout the onboarding journey.
- Act as a point of contact between newly onboarded providers and outreach managers to facilitate smooth coordination.
- Maintain and update data across multiple platforms such as a CRM, electronic medical records system, and analytics tools.
- Handle private health information with care and in compliance with confidentiality standards.
- Ensure data accuracy and consistency in system entries and support periodic data verification processes.
- Attend in-person industry events, conferences, or team gatherings when opportunities arise.
- Communicate clearly and confidently about organizational services, patient demographics, and referral procedures.
- Complete additional tasks as necessary to support team objectives.
