About the Role

International SOS Government Medical Services seeks a Physician Primary Care to deliver on-site primary care services within approved clinical privileges and scope of practice. In this role, you will serve as the team leader within the Patient Aligned Care Team model, coordinating comprehensive care for veterans.

What You'll Do

  • Provide on-site primary care services within approved clinical privileges and scope of practice.
  • Deliver comprehensive primary care, including preventive care, immunizations, cancer screening, lifestyle counseling, chronic disease management, acute care evaluation and treatment, and mental health screening.
  • Serve as the team leader in the Patient Aligned Care Team model and coordinate care with specialists, mental health providers, social workers, pharmacists, and community care providers.
  • Educate veterans on nutrition, exercise, smoking cessation, and alcohol use, and use clinical reminders and health maintenance tools in the electronic health record to track preventive care.
  • Address aging-related issues, including cognitive decline, falls, and polypharmacy, and collaborate with Geriatrics and Extended Care and Palliative Care teams when appropriate.
  • Provide women's health care, including gender-specific care, reproductive health, menopause support, and trauma-informed care.
  • Conduct telehealth and virtual care activities, including video visits, telephone appointments, secure messaging, and remote patient monitoring.
  • Review inpatient admissions recommended by an Advanced Practice Provider and ensure person-to-person handoff to the admitting team.
  • Provide consultation to referring physicians, determine the appropriate course of treatment, and communicate with referring clinicians.
  • Initiate appropriate social work referrals for identified homeless veterans and for patients who do not have primary care providers.
  • Follow all established medication policies and procedures and do not provide sample medications to patients.
  • Ensure mechanisms are in place to communicate test results to patients.
  • Maintain accurate, timely, and complete documentation in the current electronic health record, select appropriate evaluation and management procedure codes, and complete documentation and coding by the end of each patient care encounter.
  • Comply with Privacy Act, confidentiality, Freedom of Information Act, Health Insurance Portability and Accountability Act, Veterans Health Administration privacy and release of information requirements, and all applicable medical records requirements.
  • Follow all established patient safety and infection control standards of care, make every effort to prevent medication errors, falls, and patient injury, document and disclose patient safety events as required, and submit required entries in the Veterans Affairs Patient Safety Reporting System.
  • Attend staff meetings as required by the facility Chief of Service, Chief of Staff, or designee, including Primary Care all-staff meetings and provider discussions.
  • Complete appropriate Quality Management and Performance Improvement documentation related to procedures, complications, and outcomes of examinations.
  • Be present and on time for scheduled shifts, sign in and out when in attendance, and support continuity of care requirements.
  • Support timely patient access in accordance with clinic wait time and consult completion requirements.
  • Maintain current licensure, registration, certification, continuing medical education, and competency documentation, and provide renewal documentation as required.
  • Complete all required Veterans Affairs mandatory training and any additional training communicated by the facility.
  • Comply with facility Medical Staff Bylaws, rules, regulations, and professional conduct expectations.

What We're Looking For

  • Current medical licensure, registration, and certification.
  • Completion of all required Veterans Affairs mandatory training.

You will serve as team leader in the Patient Aligned Care Team model, coordinating with specialists, mental health providers, social workers, pharmacists, and community care providers. You will report to the Facility Chief of Service, Chief of Staff, or designee.
