Theoria Medical is seeking a Physician (MD/DO) to join our team. In this role, you will serve as the attending physician for residents at skilled nursing facilities, delivering patient-centered, comprehensive primary care. You will be responsible for diagnosing and treating conditions while collaborating with and supervising advanced practice providers.

What You'll Do

Serve as the attending physician for residents residing at each assigned skilled nursing facility.
Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care.
Provide high quality, patient-centered, comprehensive primary care services by rounding on patients in SNFs.
Diagnose and treat patients with acute illnesses and chronic diseases and conditions.
Complete provider documentation in Theoria’s proprietary EHR system, ChartEasy™.
Participate in Theoria’s Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.
Supervise collaborating nurse practitioners and physician assistants.
Utilize electronic/online medical tools such as EHR systems and state/county systems for administrative functions.
