What You'll Do
- Proactively reach out to patients to support their care plans and answer questions
- Serve as a primary point of contact, facilitating clear communication between patients and the care team
- Coordinate patient resources and follow-ups to ensure continuity of care
- Document patient interactions and feedback accurately in relevant systems
