Responsibilities
- In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions.
- Evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
- Serve as an educator, patient advocate, and a member of the interdisciplinary care team.

- Candidates must be currently located in, or planning to relocate to, the Richmond area (El Cerrito, San Pablo, Pinole, Hercules, Crockett).
- All opportunities require being in the field visiting patients in their homes.
- Remote or hybrid work is not available.
- Prior to scheduling an in-person interview, all candidates are required to complete a computer literacy and competency assessment.
- The assessment ensures ability to navigate electronic charting, understand company values, and use web-based communication applications.
- Company-issued iPad or Chromebook provided.
