CareHarmony is hiring an Outpatient Registered Nurse - RN for an entry-level position in our Clinical Advancement Program (CAP 1). In this role, you will be accountable for providing and coordinating clinically competent care for assigned patients within the End Stage Kidney Disease (ESKD) healthcare team. Your work will include patient assessment, planning, intervention, evaluation, and active participation in team decision-making and quality improvement activities.
What You'll Do
- Perform systematic collection and analysis of patient data before, during, and after hemodialysis treatment, documenting findings and adjusting treatment plans.
- Assess, collaborate, and document patient and family learning needs to provide initial and ongoing education.
- Direct and provide all aspects of the daily safe and effective delivery of hemodialysis therapy to assigned patients.
- Administer medications as prescribed or per approved clinical algorithms, documenting medical justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as a resource for the healthcare team and participate in staff training and the orientation of new staff.
- Ensure correct laboratory collection, processing, and shipping procedures are performed and reschedule missed collections.
- Identify expected outcomes and document or update the nursing assessment and plan of care for assigned patients through Interdisciplinary Team collaboration.
- Ensure patient awareness related to transplant and treatment modality options.
- Complete Clinical Advancement Program requirements to advance.
- Perform all other duties as assigned by your Supervisor.
What We're Looking For
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet the practice requirements in the state of employment.
- This is an entry-level position for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse.
Nice to Have
- Chronic or acute hemodialysis experience.
Team & Environment
You will work under the direction of the Supervisor with guidance from a Clinical Educator or Preceptor, or in collaboration with other RNs. After meeting experience requirements, you may have oversight of Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or designated Nurse in Charge.
