Appleton Ave. Suite B Milwaukee, WI 53222 PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is your entry point into the Clinical Advancement Program (CAP). In this role, you'll be fully responsible for delivering and coordinating top-notch patient care, which includes comprehensive assessment, planning, intervention, and evaluation for a specific patient group. You might also delegate tasks to direct patient care staff like RNs, LVN/LPNs, and Patient Care Technicians. As a key member of the End Stage Kidney Disease (ESKD) healthcare team, you'll actively participate in decision-making, teaching, leadership, and quality improvement initiatives that directly enhance patient outcomes and facility operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties will be performed in line with Fresenius Kidney Care policy, nursing practice standards, and state and federal regulations. · Work under the guidance of your Supervisor, Clinical Educator, Preceptor, or in collaboration with other Registered Nurses. · Conduct ongoing, systematic patient data collection and analysis before, during, and after hemodialysis treatments, documenting in patient medical records. Make treatment plan adjustments as needed and notify Team Leader, Charge Nurse, Supervisor, or Physician. · Assess, collaborate, and document patient/family learning needs to provide comprehensive initial and ongoing education. · Collaborate with direct and ancillary patient care staff to ensure safe and effective hemodialysis therapy delivery. · Administer medications as prescribed, documenting medical justification and effectiveness. · Initiate or assist with emergency response measures. · Serve as a healthcare team resource, participate in staff training and new staff orientation. · Ensure proper laboratory collection, processing, and shipping procedures, rescheduling missed or insufficient collections. · Identify and document patient outcomes, updating nursing assessments through interdisciplinary team collaboration. · Ensure patient awareness of transplant and treatment modality options. · Complete CAP requirements for advancement. · Perform additional duties as assigned by Supervisor. PHYSICAL DEMANDS AND WORKING CONDITIONS: These characteristics represent typical employee experiences while performing essential job functions. Equipment and coworkers can help. · Requires frequent, prolonged standing and ability to bend. · May occasionally move machines/equipment up to 200 lbs (with assistance) and lift chemical/water solutions up to 30 lbs to 5 feet high. · Healthcare facility environment with temperature control and moderate noise levels. · Potential exposure to infectious/contagious materials. · Daily work includes desk/computer tasks, patient interactions, and communication with facility/hospital staff and physicians. · May require travel to training sites or other facilities. · May perform essential functions in alternative locations with similar physical demands.
