21st Century Home Health Services Inc. is seeking an Occupational Therapist to provide direct, high-quality services to patients in their homes and communities in the San Ramon, California area. You will play a key role in delivering personalized care that enhances patient independence and quality of life.
What You'll Do
- Conduct comprehensive evaluations to assess patient needs and functional abilities.
- Develop and implement individualized treatment plans focused on therapeutic goals.
- Provide direct occupational therapy services and interventions in the field.
- Educate patients and their families on treatment plans and adaptive techniques.
- Maintain accurate and timely documentation of patient progress and outcomes.
What We're Looking For
- Current and valid California Occupational Therapist license.
- Proven experience providing occupational therapy services, ideally in a home health or community setting.
- Strong clinical reasoning and patient assessment skills.
- Excellent written and verbal communication abilities.
- Valid driver's license and reliable transportation for travel within the service area.
Work Mode
This role operates on a hybrid model based out of the San Ramon, California area. It combines in-field patient visits with administrative responsibilities.
