About the Role
The nurse practitioner will assess, diagnose, and manage acute and chronic conditions for high-acuity patients, working collaboratively with interdisciplinary teams to ensure comprehensive care delivery.
Responsibilities
- Conduct thorough patient evaluations and physical assessments
- Develop and implement individualized treatment plans
- Order and interpret diagnostic tests and lab results
- Prescribe medications and adjust therapies as needed
- Coordinate care across specialties and service providers
- Document clinical findings and progress in electronic health records
- Provide patient and family education on disease management
- Respond to urgent clinical changes in patient status
- Support transitions of care between settings
- Maintain compliance with clinical protocols and standards
- Participate in quality improvement initiatives
- Collaborate with social workers and care coordinators
- Ensure adherence to regulatory and accreditation requirements
- Conduct follow-up visits and monitor treatment outcomes
- Serve as a clinical resource for support staff
- Engage in ongoing professional development
- Uphold patient confidentiality and ethical standards
- Contribute to clinical policy development
- Assist in training new clinical team members
- Promote patient safety and risk reduction strategies
- Utilize telehealth platforms when appropriate
- Address psychosocial aspects of chronic illness
- Support end-of-life care planning when necessary
- Respond to after-hours clinical inquiries as scheduled
- Maintain current licensure and certifications
