Braze is hiring a Nurse Practitioner (Hybrid Remote) to join Strive's interdisciplinary clinical care team. You will utilize your advanced nursing skills to deliver exceptional care for CKD and ESKD patients, acting as an advocate, navigator, and trusted clinician.
What You'll Do
- Manage patient outcomes and KPIs for a panel of CKD and ESKD patients.
- Collaborate with external providers to align goals, treatment, and care plans.
- Obtain patient history, perform physical exams, order and interpret diagnostic tests, and formulate short-term and longitudinal care plans.
- Present patient cases and provide clinical support for rounds and interdisciplinary team meetings.
- Ensure care plans incorporate best practices for chronic kidney care, including CKD clinical care, ESKD support, and end-of-life/advanced care planning discussions.
- Serve patients across multiple settings including patient homes, clinics, partner MD spaces, and via telehealth.
- Act as a liaison with dialysis facilities, PCPs, and nephrologists to streamline care transitions.
- Maintain current board certification and state-specific continuing education requirements.
- Adhere to Strive's documentation policy expectations.
- Deliver primary and urgent care visits to adult and geriatric patients with chronic kidney disease in designated markets.
- Provide in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting.
What We're Looking For
- Master's degree in Nursing or a similar field.
- 2+ years experience as a Family Nurse Practitioner, Primary Care Nurse Practitioner, or experience in internal medicine, cardiology, Nephrology, or Case Management.
- Current state-specific licensure.
- National Board Certification.
- Current Drug Enforcement Administration (DEA) license or eligibility to obtain within 90 days of hire.
- Current BLS or CPR Certification.
- Internet connectivity with minimum speeds of 3.8Mbps/3.0Mbps (up/down) and latency <60 ms.
- Efficient and reliable transportation, including an active driver’s license, for travel across an assigned region.
Nice to Have
- Basic computer and Microsoft Office skills.
- 2+ years experience with Electronic Health Records (EHR).
- Wound Care certification.
- Experience using audio-visual technology platforms.

Team & Environment
You will be part of Strive’s interdisciplinary clinical care team, working with social workers, care coordinators, pharmacists, dietitians, and physicians, and reporting to the Lead, Nurse Practitioner.
