DaVita IKC is hiring a Nurse Practitioner for the Flex Team to provide integrated care for patients with complex, chronic kidney disease. You will play a key role in navigating the healthcare system to improve quality of life and health outcomes.
What You'll Do
- Complete Comprehensive Health Assessments.
- Manage complex patients, focused on reducing hospitalizations and improving health outcomes.
- Deliver holistic, integrated care to improve quality of life.
- Coordinate and facilitate quality, consistent, cost-effective care.
What We're Looking For
- Passion for delivering care to patients with complex, chronic illnesses.
- Minimum of two (2) years' experience as a Nurse Practitioner (NP).
- Current unrestricted NP license to practice in the state.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook.
- Ability to work from a Home Office with High Speed Internet.
- Willingness to obtain up to 6 additional state licensures for remote patient management.
Nice to Have
- Experience in Value Based Care or Population Health, including documenting/coding from a Medicare Advantage risk perspective and conducting Health Risk Assessments.
- 5+ years' experience as a Nurse Practitioner practicing with adult/gerontology populations.
- Spanish-speaking ability preferred in some markets.
- Experience with longitudinal management of chronic illnesses (CHF, DM, CV disease, HTN).
