Provide advanced clinical care to patients with complex kidney conditions as part of an integrated health team in Des Moines, IA. In this hybrid remote role, you’ll conduct comprehensive health evaluations, manage chronic kidney disease (CKD) and end-stage kidney disease (ESKD), and support patients across medical, emotional, and social dimensions. Your work will directly contribute to reducing hospitalizations, improving long-term outcomes, and advancing value-based care models.
Key Responsibilities
- Perform thorough health assessments to identify and manage comorbid conditions
- Prescribe medications, interpret diagnostic results, and adjust treatment plans as needed
- Collaborate with nephrologists and care teams to align clinical interventions with patient goals
- Address psychosocial and financial barriers that impact health outcomes
- Coordinate care across settings, including dialysis clinics and outpatient practices
- Maintain a flexible, self-directed schedule that supports patient and partner needs
Qualifications
You bring at least two years of experience as a licensed Nurse Practitioner, with strong clinical judgment and the ability to work independently. You must hold an unrestricted state license, DEA eligibility or registration, and current CPR/BLS certification. Proficiency in electronic health records, particularly Cerner or similar systems, is essential. A reliable vehicle, valid driver’s license, and home office setup with high-speed internet are required.
Preferred candidates have five or more years in primary or specialty care—especially in nephrology, internal medicine, or cardiology—and experience in population health, Medicare Advantage, or health risk assessments.
Compensation and Benefits
This full-time position offers a salary range of $118,000 to $150,000 per year, with a comprehensive total rewards package.
