Vynca is hiring a Nurse Practitioner to serve as a vital member of our Clinical Care Delivery Team. In this role, you will provide evidence-based, high-quality clinical care via telemedicine to an assigned panel of complex and seriously ill patients.
What You'll Do
- Independently perform comprehensive and symptom management visits to address patient needs and oversee plans of care.
- Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs.
- Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
- Interface with patients, families, caregivers, and clinical staff to ensure customer satisfaction and clinical outcomes.
- Facilitate the patient’s transition within and between health care settings.
- Collaborate with multidisciplinary team members by making appropriate referrals.
- Facilitate staff, patient, and family decision-making by providing educational tools.
- Understand Medicare reimbursement and coding for all levels of service in addition to Medicaid and other regulatory requirements.
- Actively participate in any mandatory meetings of the organization.
- Participate in an after-hours on-call rotation.
What We're Looking For
- Current, unrestricted California RN and NP license, with the ability to obtain additional state licensure.
- DEA licensure, or the ability to obtain.
- Graduate of an accredited nurse practitioner program.
- ANP, AGNP, or FNP Certification.
- 2+ years experience providing NP care to complex, seriously ill patients.
- Ability to work in a Remote environment, Monday-Friday 8:30am-5:00pm PST, with rotating evening/weekend on-call coverage.
Nice to Have
- Additional Oregon RN & NP licensure.
- ACHPN certification.
