St. Mary's Medical Center is hiring an Oncology Nurse Navigator to serve as a dedicated patient advocate and care coordinator. You will guide cancer patients through their treatment journey, from diagnosis to survivorship or end-of-life care.
What You'll Do
- Provide individualized education and emotional support.
- Coordinate multidisciplinary care and appointments.
- Identify and address barriers to care.
- Connect patients with community and financial resources.
- Facilitate communication between patients, families, and care teams.
- Track patients throughout their care continuum to prevent care gaps.
