Mountainlands Community Health Center Inc is looking for a Mental Health Therapist to join our mission-driven team. You will provide individual, couples, and family counseling services to clients across the lifespan, ensuring comprehensive, coordinated care within our Patient Centered Medical Home.
What You'll Do
- Conduct biopsychosocial assessments and develop individualized treatment plans
- Provide individual, couples, and family therapy
- Diagnose mental health conditions in accordance with DSM-5-TR criteria
- Maintain timely and accurate clinical documentation in the electronic medical record (EMR)
- Provide crisis intervention and risk assessment as needed
- Collaborate with primary care providers and other team members in an integrated care model
- Participate in case conferences, team meetings, supervision, and peer/chart reviews
- Support care coordination and referrals to community resources
- Comply with all ethical, legal, and regulatory requirements
What We're Looking For
- Master's degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy
- Current licensure (e.g., LCSW, LMHC, LMFT) or eligibility and active pursuit of licensure, if applicable
- Proficient English and Spanish communication skills
Nice to Have
- Comfortable working with a range of individuals from different ages (children, teenagers, adults), diagnostic conditions, and other diverse populations
- Knowledge of trauma-informed care and evidence-based treatment modalities
- Experience working with individuals experiencing substance use disorders
- Strong documentation and organization skills
