Millennium Physicians Group (MPG) is hiring a Memory Support Navigator to act as a vital bridge between primary care and the community. This role serves as the dedicated liaison for families and caregivers navigating the complexities of a dementia diagnosis, ensuring they are supported throughout their journey.
What You'll Do
- Act as the dedicated point of contact for families referred by primary care providers, offering emotional support and expert guidance.
- Identify, vet, and maintain a robust database of local and national dementia resources, including adult day programs, respite care, and memory cafes.
- Help caregivers navigate the healthcare system and social services, ensuring they understand their options for home health, long-term care, and financial assistance.
- Connect caregivers with existing support groups to reduce caregiver burnout and isolation.
- Assist families interested in clinical trials by locating relevant dementia research studies and explaining the participation process.
- Work closely with primary care physicians to update them on the social and environmental needs of their patients, ensuring a holistic approach to treatment.
What We're Looking For
- High School Diploma.
- A deep, genuine empathy for individuals living with cognitive impairment and the families who support them.
Nice to Have
- 2+ years of experience working directly with dementia patients and their caregivers.
- Proven ability to navigate community-based resources and a strong understanding of the continuum of care (home care, assisted living, hospice).
- Exceptional active listening skills and the ability to explain complex medical or legal concepts in a supportive, easy-to-understand manner.
