Community Health Network is looking for a Medication Reconciliation Technician to join our team. The technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner, in compliance with standard procedures, the Joint Commission, and other regulatory standards. What You'll Do
- Interpret and enter 'Prior to Admission' medication history into the EPIC electronic medical record.
- Perform thorough interviews with patients and their family members.
- Conduct telephone interviews with outpatient pharmacies, long-term care facilities, or other sources to obtain medication history.
- Track interventions and productivity using the department's established process.
- Communicate regularly and professionally with patients, families, and hospital staff.
What We're Looking For
- High School diploma or GED equivalent.
Nice to Have
- Previous pharmaceutical or related healthcare experience.
- Previous medication history taking experience.
