Centene Management Company LLC is looking for an LTSS Service Care Coordinator to join our mission of transforming community health. In this role, you will develop, assess, and coordinate holistic care management activities to enable quality, cost-effective healthcare outcomes for Medicaid elderly and physically disabled members.
What You'll Do
- Develop, assess, and coordinate holistic care management activities to enable quality, cost-effective healthcare outcomes.
- Develop or assist with developing personalized service care plans for long-term care members.
- Educate members and their families/caregivers on available services and benefits.
- Evaluate member needs and available resources to recommend and facilitate the best outcome plan.
- Assist with developing ongoing long-term care plans and identify needed providers, specialists, and community resources.
- Coordinate between the member, family/caregivers, and the care provider team to ensure services are accessible.
- Provide resource support for various needs (e.g., employment, housing, independent living) based on assessments.
- Monitor care plans, member status, and outcomes, providing recommendations based on identified needs.
- Interact with long-term care healthcare providers and partners to ensure member needs are met.
- Collect, document, and maintain member information and care management activities for regulatory compliance.
- Perform on-site visits to assess member needs and collaborate with providers or resources.
- Provide and facilitate education to members and families on procedures, provider instructions, options, and benefits.
- Provide feedback to leadership on opportunities to improve quality of care and service delivery.
What We're Looking For
- A Bachelor's degree and 1 year of related experience, OR equivalent experience acquired through applicable knowledge, duties, scope and skill.
- For the Illinois Plan (Physically Disabled/Elderly): Must meet one of three criteria: 1) RN licensed in Illinois. 2) Bachelor or Master’s Degree prepared in a specified human services field. 3) LPN with one (1) year experience in comprehensive assessments and provision of formal service for the elderly.
