Ochsner Health is looking for an LPN for a locally remote post-discharge opportunity. You will assist in data collection and the implementation of healthcare services, engaging patients to maintain optimal health and minimize acute issues related to chronic conditions. What You'll Do
- Oversee the disease registry database.
- Conduct pre-visit chart reviews for patients.
- Coordinate care across the continuum.
- Participate in quality improvement activities.
What We're Looking For
- Graduate of an accredited school of practical nursing.
- 2 years of ambulatory care experience.
- Current LPN license in the state of practice.
Nice to Have
- 3 years of combined ambulatory care and home health care experience.
