Corewell Health is hiring a Licensed Practical Nurse (LPN) for our Long Term Care facility. In this role, you will provide essential care to residents on sub-acute, chronic care, and dementia care units, working under the direction of an RN, mid-level provider, or physician.
What You'll Do
- Contribute to the assessment, planning, implementation, and evaluation of patient care.
- Inform patients and families about treatments, medications, and equipment being utilized.
- Document care provided via computer and written patient record consistent with guidelines.
- Work closely with doctors, social work, dieticians, PT/OT, hospice, and spiritual support.
- Provide care to residents with multiple clinical issues, including CHF, dehydration, cellulitis, diabetes, DVT, and stable cardiac issues.
- Provide and implement treatment plans for long-term care and rehabilitation residents.
- Work on a secured dementia unit to create a fullness of life for residents.
What We're Looking For
- Graduate of an approved LPN program.
- LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan upon hire.
- CRT-Basic Life Support (BLS) - AHA American Heart Association upon hire.
Nice to Have
- Experience in Skilled Nursing or Long Term Care.
