Advanced Health Care of Albuquerque is hiring a Licensed Practical Nurse (LPN) to deliver high-quality, patient-centered nursing care within our skilled nursing facility. In this role, you will work under RN supervision to assist in managing care for residents recovering from surgery or managing chronic illnesses.
What You'll Do
- Administer medications and treatments as prescribed and in accordance with facility protocols.
- Perform routine and specialized nursing tasks, including wound care, catheter insertion, and monitoring vital signs.
- Assist in developing and implementing individualized care plans with the resident, family, and healthcare team.
- Monitor, document, and report resident progress and changes in condition to the supervising RN.
- Educate residents, families, and caregivers about facility services and care options.
- Assist with resident mobility, transfers, and activities of daily living as needed.
- Maintain accurate and up-to-date resident records compliant with regulatory standards.
- Collaborate with RNs, physicians, and other healthcare professionals to ensure coordinated care.
- Respond promptly to resident needs and changes in condition with appropriate interventions.
- Adhere strictly to infection control and safety procedures.
- Perform all other duties assigned by the supervisor.
What We're Looking For
- Valid Licensed Practical Nurse (LPN) license in the state where work is performed.
- At least 1 year of nursing experience.
- Strong clinical skills and precision in performing a range of nursing procedures.
- Excellent communication and interpersonal skills for effective interaction with residents, families, and professionals.
- Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt in a fast-paced environment.
- A compassionate and caring demeanor to build rapport with residents.
- Ability to lift and transfer patients up to 50lbs, with or without assistance.
- Capability to perform physical tasks like bending, standing, walking, and climbing stairs for extended periods.
- Strength to handle medical equipment and assist with mobility.
- Visual acuity and dexterity for tasks like reading records, administering injections, and handling instruments.
- Ability to respond swiftly and efficiently to emergency situations.
- Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.
- Commitment to regular attendance and timeliness as set by the department schedule.
- Ability to accept criticism and deal calmly and effectively with high-stress situations.
- Core strength to support the body repeatedly or continuously without fatigue.
Nice to Have
- Experience preferably in a skilled nursing facility, long-term care, or related fields.
Team & Environment
You will work under the supervision of a Registered Nurse (RN) and collaborate with RNs, physicians, and other healthcare professionals to deliver coordinated and comprehensive care.
